Frequently Asked Questions
Seniors who qualify for PASSPORT or Summit County Aging in Place do not have an enrollment fee to participate in our adult day care center.
You qualify for PASSPORT if you are:

Age 60 or older;

Financially eligible for Medicaid institutional care (for 2024, this means typically earning no more than $2,829 per month for one person and having no more than $2,000 in countable assets for one person, though individuals above this limit may be eligible based on the extent of their medical and in-home needs);

Frail enough to require a nursing home level of care; and
-
Able to remain safely at home with the consent of their physician.
You qualify for Summit County Aging in Place if you are:

Age 60 or older;
-
A resident of Summit County;

Not eligible for Medicaid; and
-
Enrolled in the Summit County Aging in Place program (call 877-770-5558 or visit www.dhad.org/refer and request a free in-home assessment to start the process of enrollment)
The adult day care center has programming on Tuesdays and Thursdays, from 9:30AM-2:30PM. Seniors will have time to socialize, use the facility’s exercise equipment and entertainment activities, and participate in education sessions relating to health, dementia management, self-care, English, among other topics. They will also receive a hot lunch, fruit, and snacks. Those who require transportation will be picked up from their homes with a JHC van along with other seniors who request pickup.
