Frequently Asked Questions

  • Seniors who qualify for PASSPORT or Summit County Aging in Place do not have an enrollment fee to participate in our adult day care center.

  • You qualify for PASSPORT if you are:

    • Age 60 or older;

    • Financially eligible for Medicaid institutional care (for 2024, this means typically earning no more than $2,829 per month for one person and having no more than $2,000 in countable assets for one person, though individuals above this limit may be eligible based on the extent of their medical and in-home needs);

    • Frail enough to require a nursing home level of care; and

    • Able to remain safely at home with the consent of their physician.

    You qualify for Summit County Aging in Place if you are:

    • Age 60 or older;

    • A resident of Summit County;

    • Not eligible for Medicaid; and

    • Enrolled in the Summit County Aging in Place program (call 877-770-5558 or visit www.dhad.org/refer and request a free in-home assessment to start the process of enrollment)

  • The adult day care center has programming on Tuesdays and Thursdays, from 9:30AM-2:30PM. Seniors will have time to socialize, use the facility’s exercise equipment and entertainment activities, and participate in education sessions relating to health, dementia management, self-care, English, among other topics. They will also receive a hot lunch, fruit, and snacks. Those who require transportation will be picked up from their homes with a JHC van along with other seniors who request pickup.
